The Real-Time Alert form is a resource for faculty members and staff who work with students to alert the academic advising team of any concerns they may have regarding a student, whether it be poor attendance, a change in attitude, a drop in grades, or something else.
This form can be used at any time during the academic year, or in "real time" that the situation has occurred or is occurring. Use of this form will initiate an immediate response/contact by Mike Verhagen, senior DUS programs coordinator, or a designee to work with the student in addressing emerging situations. This will enable the advising team to predict needed resources and referrals. Submitters will be provided a summary of action taken.
